Assists with maintaining schedules for medication--delivery, administration, etc. 6. Provide 20 hours medically necessary BLS per month and ensure other shift members do the same documenting and billing per State Standards and Center Policy. Ensure that BLS notes are filed in chart. 7. Divide chores equally between shift workers and ensure chores are completed by end of shift. 8. Ensure each person clocks in and out on time without undue overtime. 9. Point of contact for the on-call nursing staff for all routine and non-routine issues maintaining open communication with nursing staff. 10. Point of contact with Residential Supervisor for any issues, problems, consumer care needs, etc. 11. Use professional judgment to make appropriate decisions until Supervisor may be notified and further instructions received in an event of a non- routine matter. 12. Ensure meals/ snacks follow the posted menu. Make sure any necessary changes are approved, substituting food only when the posted food is not available. Ensure servings are appropriate, plentiful, but not wasteful. Ensure that all meals are cooked fresh not in advance. 13. Report any concerns and problems, consumer related or staff/policy/center related to the supervisor either immediately for a crisis or the next day for non emergencies. 14. Ensure all staff utilizes the pass along book to communicate. Ensure staff on the shift does not remove pages or entries from the pass along book. 15. Verbally communicate any shift issues/needs/consumer care issues to next lead house manager daily. Complete any check lists or forms as necessary. 16. Provide timely statistical data as required by the center. 17. Complete relevant extra job duties and special projects as directed in a timely manner. 18. Participates in required training. 19. Provides medically necessary services for Hillwood which includes medication monitoring, patient assistance procurement, prior authorizations, administration of injections, crisis intervention, mental health consultation, family support and educational services, meeting requirements for direct service and assigned productivity standards. 20. Consults with psychiatrist regarding medication and medical procedures. 21. Assists staff psychiatrist with follow-up services for residential consumers. 22. Oversees lab services and maintain supplies. 23. Serves as liaison with pharmaceutical representatives and other healthcare professionals regarding consumer treatment as delegated by MAS RN. 24. Assesses needs that contribute to the individualized service plan of residential consumers. 25. Provides medications to consumers to include injections. 26. Assists Residential RN with any other relevant job duties. 27. Assists with other Mental Illness offices as needed. 28. Assists with preparation for MD appointments, including transporting and attendance at MD appointments to facilitate optimal consumer services for Hillwood Group Home residents.

ADMINISTRATIVE 1. Adheres to DMH standards related to the Board of Nursing’s Nurse Delegation standards at all times. 2. Participates in community consultation and public education activities as assigned. 3. Participates in supervision as required by DMH Standards and/or assigned by supervisor. 4. Attends training and education at workshops as required, including Consumer Protection and Crisis Intervention with assaultive consumers. 5. Participates in Performance Improvement/Quality Assurance Reviews. 6. Provides on-call service as required. 7. Maintains consumer records documentation according to Center requirements and DMH standards. 8. Prepares written reports, forms and other documentation as assigned. 9. Job Description:

LOCATION OF DUTIES: Assigned Adult Day Habilitation Program, Adult Residential Facility, with travel required via program vehicle to provide transportation to people for services and/or supports related to habilitation.

MINIMUM QUALIFICATIONS: Graduation from an approved school of practical nursing. Licensed as a Practical Nurse in the State of Alabama. Certification in First Aid and CPR. Valid Alabama Driver's License, a safe driving record and insurability. Physical ability to team lift/transfer adult persons with limited mobility. Ability to drive a mini-van. Willingness to work in non-traditional and in-home settings.

PREFERRED EXPERIENCE/KNOWLEDGE: One year prior experience in direct services for persons with developmental disabilities.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Knowledge and understanding of developmental disabilities. Ability to establish and maintain positive working relationships with consumers, families, co-workers, supervisors, DMH personnel, physicians and social serive agencies. Abillity to work independently and make sound judgements regarding people's care and is resourceful and calm in emergencies. Ability to plan and organize daily schedule. Ability to work a flexible schedule. Knowledge of effects, side effects and adverse reations to meedications, including psychotropic medications. Knowledge of available medical and social community resources.
